AGREEMENT:
I understand that all players must be dropped off and picked up promptly and that YLT does not provide any supervision before or after the scheduled arrival/pick-up time at my league site. I agree to return a signed Waiver/Assumption of Risk/Indemnity/Emergency Release Agreement, and accept that without this signed form my child will not be allowed to participate in any YLT programs. I further attest that all of the info I have enclosed is correct and truthful to the best of my knowledge. I agree to pay a $15.00 fee for any returned check. I understand that NO refunds will be given AFTER the refund deadline posted on the YLT website for ANY reason. If I wish to withdraw from the league & receive a refund, I will withdraw in writing via mail, fax or email by the refund deadline. I understand that there is a nonrefundable $25.00 registration fee. GOVERNING LAW. This Agreement shall be governed by and construed in accordance with the laws of the State of California. The parties hereby irrevocably consent to the jurisdiction of the state and federal courts located in Van Nuys, California, in any action arising out of or relating to this Agreement, and waives any other venue to which either party might be entitled by domicile or otherwise. ATTORNEY'S FEES. If any action at law or in equity is brought to enforce or interpret the provision of this Agreement, the prevailing party in such action shall be awarded its attorneys' fees and costs incurred.

REGISTRATION FEES: Checks and Money Orders should be made payable to Youth League Tennis. All session fees include balls, t-shirt, progress report, trophy & 8 classes.

Registration Fees vary depending upon how early you register. Fee schedule is listed on the YLT schedule page.	$135; $145; or $155

USTA Registration Fee: Includes Jr. Membership & 1 yr. subscription to Tennis Magazine, plus local tennis newsletter and magazine & discount tickets to local pro events.	$ 18.00

REFUND POLICY If you wish to withdraw from the league & receive a refund, you must withdraw in writing via mail, fax or email by the refund deadline . There is a non-refundable $25.00 cancellation fee, in the event of a refund request. You can email a refund request to laura@youthleaguetennis.org or fax: 818-340-9171 You will automatically receive a FULL refund if: 1. you register to play at a site that is full; or 2. the level of players at your site, does not match your child's playing level, per your YLT coaches assessment. Refunds will be processed after the league begins. NO refunds are issued for ANY reason after the refund deadline once you have been registered. There are NO refunds due to time changes. Times are subject to change. There are NO refunds for rain outs or natural disasters. (YLT provides to 2 make ups are provided per season for rain outs.)
