AGREEMENT:
I understand that all players must be dropped off and picked up promptly and that YLT does not provide any supervision before or after the scheduled arrival/pick-up time at my league site. I agree to return a signed Waiver/Assumption of Risk/Indemnity/Emergency Release Agreement, and accept that without this signed form my child will not be allowed to participate in any YLT programs. I further attest that all of the info I have enclosed is correct and truthful to the best of my knowledge. I agree to pay a $15.00 fee for any returned check. I understand that NO refunds will be given AFTER JUNE 12, 2007 for ANY reason. If I wish to withdraw from the league & receive a refund, I will withdraw in writing via mail, fax or email by JUNE 12, 2007. I understand that there is a nonrefundable $25.00 registration fee. GOVERNING LAW. This Agreement shall be governed by and construed in accordance with the laws of the State of California. The parties hereby irrevocably consent to the jurisdiction of the state and federal courts located in Van Nuys, California, in any action arising out of or relating to this Agreement, and waives any other venue to which either party might be entitled by domicile or otherwise. ATTORNEY'S FEES. If any action at law or in equity is brought to enforce or interpret the provision of this Agreement, the prevailing party in such action shall be awarded its attorneys' fees and costs incurred

REGISTRATION FEES: Checks and Money Orders should be made payable to Youth League Tennis. All session fees include balls, trophy, progress report, t-shirt & 8 classes.

Players must provide their own tennis racquets. (Great Deals offered at The Sports Authority & Sportmart) YLT recommends Wilson racquets.

You can become a Jr member of the USTA when you register to play YLT. $18.00 includes a Junior membership for 1 year, a subscription to the USTA newsletter, Tennis magazine & Inside Tennis and discount tickets to local tennis events. Players must be members of the USTA before they can compete in sanctioned So Cal tournaments.	$ 18.00
REFUND POLICY If you wish to withdraw from the league & receive a refund, you must withdraw in writing via mail, fax or email by JUNE 12 , 2007. There is a non-refundable $25.00 cancellation fee, in the event of a refund request. You will automatically receive a FULL refund if: 1. you register to play at a site that is already full when you register; or 2. the level of players at your site, does not match your child's playing level, per your YLT coaches assessment. Refunds will be processed after the league begins. NO refunds are issued for ANY reason after JUNE 12 , 2007, once you have been registered. There are NO refunds due to time changes. Times are subject to change. There are NO refunds for rain outs or natural disasters. (YLT provides up to 2 make ups per season for rain outs.)
